Preparing for Your Visit

Whether you are coming to the hospital to stay (inpatient) or for a same-day procedure or test (outpatient), there are things that you will want to bring with you. Some items you will need:

• Current medication (either a list or the actual medicines)
• List of any allergies
• Identification (Photo ID)
• Insurance Cards
• Orders – if your medical provider has given them to you
• Proof of guardianship for pediatric patients
• Power of Attorney information
• Advanced Directives
