Diabetic education is extremely important for those living with this condition. Good Samaritan's Outpatient Diabetic Education Center is staffed with nurses who have been extensively trained to provide accurate, up-to-date, individualized education and information to best meet your needs. We understand this disease can be overwhelming and we want to help you take control of your condition so you can live your best, healthiest life.

Our diabetes education program consists of an approved curriculum focusing on:

  • The diabetes process
  • Incorporating nutrition management into your lifestyle
  • Incorporating physical activity into your lifestyle
  • Using medications safely
  • Testing and monitoring blood glucose
  • Preventing acute complications
  • Preventing chronic complications
  • Addressing individual issues and concerns
  • Developing strategies to promote a healthier lifestyle

Consultations

  • A referral from your primary care provider is required
    • Must have a diagnosis of diabetes or chronic kidney disease
  • Bring your support person
    • You will receive a lot of helpful information
  • Allow enough time for your appointment
    • Initial appointments last 1.5 - 2 hours
    • Follow-up visits usually last 30-45 minutes
  • Bring your glucose meter and log sheets
  • Bring your medications
    • Bottles are best
    • Most up-to-date medication list
